Suicide Risk Assessment Tools
Suicide risk assessment tools may be useful for determining a person's risk of suicide. Although these tools may be useful for determining an individual's risk, they should not be used as the sole determinant of suicide risk. Rather, the specific content of each assessment tool can help you decide whether an individual is at low, moderate or high risk of suicide.
Certain tools are useful for practitioners in a primary care setting. Tools that may be useful for pharmacists include:
SAD PERSONS
-Useful for screening individuals at risk of suicide
-Variety of healthcare settings (psychiatric, non-psychiatric)
-Adults
-Easy to conduct (only 10 items)
TASR (Tool for Assessment of Suicide Risk)
-Useful for screening individuals at risk of suicide
-Variety of healthcare settings (psychiatric, non-psychiatric)
-Adults, children & youth
